I consent to having photos taken of myself and / or my children I grant KMDT full right to use the images resulting from the photography / video filming, and any reproductions or adaptations of the images for fundraising, publicity or other purposes to help achieve the group’s aims. This might include (but is not limited to), the right to use them in their printed and online publicity, social media, press releases and funding applications. Waiver of Liability. This agreement releases KMDT from all liability relating to injuries that may occur during training / activities at any of KMDT’s classes / locations / events. By signing this agreement, I agree to hold KMDT entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved in Krav Mage. These include but are not limited to minor sprains, broken bones, death, etc. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity. By selected this tick box, I forfeit all right to bring suit against KMDT for any reason. In return, I will receive training in Krav Maga. I will also make every effort to obey safety precautions al listing in writing and as explained to me verbally, I will ask for clarification when needed. I agree that everything I have provided is up-to-date.

30 days cancellation notice required. Membership can be cancelled by KMDT at anytime where we feel it is necessary as a business.
